Why talk about mistakes?

According to McKinsey (2024), AI projects fail mainly for organizational—not technical—reasons. Agentic AI amplifies risk because it acts autonomously, uses multiple tools, and chains many decisions. Understanding the common errors helps prevent production incidents.
